Every resident at AleraCare has an Individual Service Plan built around their specific needs. We specialize in supporting residents managing mental illness, behavioral health conditions, substance use recovery, physical disabilities, and reentry from incarceration.

Individualized Residential Care

Specialized Support Programs

Safe, structured, and compassionate living environments tailored to the unique goals and dignity of every adult we serve.

Behavioral Health

Behavioral & Mental Health Support

At AleraCare, LLC, we believe everyone deserves a place where they feel safe, understood, and supported. We provide a warm, stable environment for individuals living with mental illness and behavioral challenges, where compassionate staff are there each day to help with structure, medication support, behavioral guidance, and calm, caring de-escalation when it's needed most.

We also encourage connection through community activities and offer hands-on help with daily living, all guided by each person's individualized service and behavior support plan. Our hope is simple: to help reduce crises, build lasting stability, nurture independence, and support a richer, more fulfilling quality of life for every individual we serve.

Active Recovery

Alcohol & Drug Dependency Support

Recovery is a journey, and no one should have to walk it alone. AleraCare, LLC offers a safe, structured, and supportive home for individuals working through drug or alcohol dependency. Our caring team walks alongside residents as they follow their treatment plans, attend counseling or recovery programs, manage medications, steer clear of triggers, and build healthy new routines.

We keep a watchful, encouraging eye on each person's progress, offer steady support and supervision, and coordinate closely with healthcare and behavioral health professionals. Above all, we're committed to helping residents grow their independence, strengthen their coping skills, and build the lasting habits that support long-term recovery.

Reentry & Transition

Support for Justice-Involved Individuals

Coming home after time in the correctional system can feel overwhelming, and having the right support can make all the difference. AleraCare, LLC provides a safe, structured, and welcoming living environment for individuals navigating this transition.

Our team is here to help residents stay on track with community supervision requirements, attend appointments and treatment programs, build positive daily routines, manage medications, and avoid substance use and other setbacks. We also focus on developing practical independent living and community skills so residents feel confident moving forward and building a successful path back into community life.

Accessible Care

Support for Adults with Physical Disabilities

Every person deserves a home where they feel comfortable, respected, and free to live life on their own terms. AleraCare, LLC offers a safe, accessible, and supportive living environment for adults with physical disabilities, thoughtfully tailored to each individual's unique needs.

Our caring staff are there to help with personal care, mobility, meals, medications, transportation, and everyday activities, always with an eye toward encouraging independence and staying connected to the community. At the heart of everything we do is a commitment to dignity, safety, independence, and a genuinely good quality of life for every person we support.

Scope of Care

A Note on What We're Not

AleraCare is a residential, non-institutional care setting. We are not a nursing home or hospital. We provide care above the level of room and board, strictly within the limits of Wisconsin's Adult Family Home licensing (DHS 88), which allows for supportive, non-intensive nursing assistance in a homelike setting.
